Responsibilities:
* Speak with clients either through phone calls follow up
* Attend outreach events
*Assist with in-take form and benefits screening
*Assist Medical Transition workshop
*Deliver health information
*Follows up with clients in an efficient and timely manner
*Assist in health education sessions and evaluations
*Research health information

Qualifications:
*Interested in the Developmental Disabilities field
*Excellent interpersonal relationship skills including oral and written communication skills
*Ability to communicate effectively over the telephone, in writing, and in-person to a variety of stakeholders
*Willingness to learn to convey Disabilities Resources
