Community Psychiatry Division
SSI Outreach Counselor- Full Time/Days
Description:
The Division of Community Psychiatry is seeking an experienced individual to help mentally ill homeless individuals apply for Social Security disability benefits.
Responsibilities:
Completes Social Security applications and supporting forms.
Writes medical summaries and psychiatric evaluations
Contacts medical providers for supporting documentation and medical evidence.
Engages clients in community settings to conduct outreach.
Accompanies clients to Social Security appointments.
Maintains client charts, including service plans, and progress notes.
Provides limited case management to help clients with issues related to housing and other immediate needs.
Completes accurate documentation in compliance with regulatory agencies.
Presents educational sessions about the SSI Project to staff and clients at collaborative agencies.
Qualifications:
Bachelor’s Degree in social work, psychology, or a related field is preferred.
A minimum of one year experience in mental health services.
Experience with those having serious and persistent mental illness.
Current, valid MD state driver’s license is required. Use of employee’s own personal vehicle is required.
Knowledge of community resources.
Effective oral and written communication and counseling skills are necessary. Must be able to demonstrate strong writing skills at the time of interview.
Clinical knowledge of mental disorders preferred.
Computer skills for reporting and tracking of data including Microsoft Word and Excel.
Must be extremely organized, able to work in a fast paced setting, and meet all deadlines.
CPR certification within 3 months of hire.
